All Students must have a completed Studen tWaiver Form on file before participation. Click on link below for the form.

Please send a note to the teacher each day your child will participate. Click on link below for the form.
After School Program Note To Teacher

Please complete the following:

*Please make checks payable to OHS PTA.

*A parent, guardian, or caregiver is needed to volunteer at every

 class. Volunteers check in at the "after school  activities 

 coordinator desk outside the library to sign in upon arrival at

 school.

*The ladies in the OHS main office cannot answer any After  

  School Program questions. Please contact Susan Ephron or Dana

  Kery with any questions or concerns.
