Please include the following:

• A completed Enrollment/Emergency form including your preference for volunteer dates
• A check payable to OHS PTA

Place in an envelope labeled “OHS After School Activities”.

If you have any questions, comments or ideas for future programs, please do not hesitate to call us.  Additionally, please call if you would like to help us out!

Thanks and remember to register early to avoid being shut out of a program!
